Mark Masons Regular Giving Form
The Mark Benevolent Fund template form (below) has two pages. The first is for the MBF and the other for your Bank.
After printing (or obtaining a printed copy from perhaps your lodge secretary) you may complete them by hand and send each on to MBF and Bank separately.
Alternatively, with a reasonable camera phone attach a photograph of the first page to an email to the MBF charitiesfinance@mmh.org.uk. Their email address is also on the form. The second page by perhaps similar means or by creating a Standing Order through your banking app.
Please in all cases ensure the mandatory fields are completed. In particular your MMH Membership No as the Reference. The MBF use this to match your form and the recieved funds to you and thus to your Lodge. Please also sign and confirm Gift Aid if that is appropriate to your circumstances.
Your generosity, is always very much appreciated.
